Service
Home Support Coordination
Home nursing, laboratory visits, physiotherapy, and caregiver support arranged through selected partners — with the limits stated plainly.
What this service helps with
Where care needs to continue at home, we identify a suitable partner provider, arrange the visit or schedule, confirm what it costs, and follow up on whether it happened and what changed.
Who it is for
- Patients recovering at home after admission or surgery
- Older adults needing regular support at home
- Families managing chronic conditions between clinic visits
- Diaspora relatives arranging home support remotely
Situations we see
Wound dressing is needed twice a week and the family cannot keep travelling to a facility.
Blood tests are due but leaving the house is difficult.
Physiotherapy was recommended and nobody has arranged a therapist.
What BRIDGE will do
- Identify suitable partner providers for the need
- Arrange visits, schedules, and repeat appointments
- Confirm known partner rates before you commit
- Coordinate lab sample collection where offered
- Follow up on completion and record what happened
- Raise concerns with the treating provider where appropriate
What BRIDGE will not do
- Employ or clinically supervise home-care staff
- Provide nursing, physiotherapy, or personal care ourselves
- Guarantee partner availability in every location
- Take responsibility for a partner provider’s clinical decisions
What we need from you
- Membership, identity, and consent in place
- The clinical instruction or care plan where one exists
- The home address and access details once the visit is agreed
- A named person at home who can receive the visit
- Agreement on the partner and rate before booking
How fees work
BRIDGE charges a coordination fee. Partner providers charge their own rates, which we confirm to you before anything is booked.
Response standard
Availability varies by location and service type. We give you a realistic timeframe rather than an optimistic one.
We aim to respond to enquiries within 30 minutes to 24 hours. Staffed hours are still being finalised. [CONFIRM BEFORE LAUNCH] Staffed hours and tier-specific response windows.
Clinical boundary
Home services are delivered by independent licensed partners. They remain responsible for their own clinical care. BRIDGE is not an emergency service. For emergencies, call 112 or go to the nearest emergency facility.
Questions
Are your partners licensed?
We coordinate through vetted or selected partners where available, against a documented verification standard.
Can I choose the provider?
Yes, where a suitable option exists in your area, and where you prefer a provider you already trust.
Do you cover the whole country?
[CONFIRM BEFORE LAUNCH] Covered locations. We do not claim national coverage before it is true.
