Service
Care Navigation
When you know something needs attention but not where to begin, this is where to start. We work out what the next practical step is and what it will involve.
What this service helps with
Care Navigation is the entry service. A coordinator listens to the situation, clarifies what is being asked, identifies which type of service or provider fits, and sets out the realistic options with what is known about access, location, and cost.
Who it is for
- People unsure which service or provider is appropriate
- Caregivers arranging care for a parent or relative
- Anyone who has been told to "see a specialist" without knowing how
- Members who need a plan before committing to appointments
Situations we see
A parent has been told they need a scan, but nobody explained where to get it or what it costs.
A test result mentions a specialist, and the family does not know which one or how to reach them.
Care has been happening in three places and nobody has the full picture.
What BRIDGE will do
- Clarify the request and confirm consent
- Identify appropriate service categories and providers
- Present options with known access routes and costs
- Explain what is realistic and what is not
- Set out the next step in writing
- Hand over to a specific service where one fits
What BRIDGE will not do
- Diagnose, interpret results, or advise on treatment
- Recommend a provider based on hidden commercial incentives
- Publish or share our private provider contact records
- Promise availability or price on a provider’s behalf
What we need from you
- Membership, identity, and consent in place
- A short description of the situation
- Location and how far you can reasonably travel
- Any referral letter or report you already hold
- A realistic sense of your budget
How fees work
Care Navigation is charged as case work. The fee reflects the time to clarify, research, verify options, and document the recommended next step. Provider charges are separate and paid to the provider.
Response standard
A navigation case is acknowledged with a reference and a response window. Complex situations take longer to research honestly than simple ones, and we say so up front.
We aim to respond to enquiries within 30 minutes to 24 hours. Staffed hours are still being finalised. [CONFIRM BEFORE LAUNCH] Staffed hours and tier-specific response windows.
Clinical boundary
Navigation is not a clinical assessment and does not replace seeing a clinician. BRIDGE is not an emergency service. For emergencies, call 112 or go to the nearest emergency facility.
Questions
Can you tell me what is wrong with me?
No. That is a clinical assessment and it belongs with a licensed provider. We help you reach the right one.
Will you give me a list of hospital contacts?
We present the options relevant to your case and coordinate with them on your behalf. We do not publish our provider contact records as a directory.
What if BRIDGE cannot help?
We tell you plainly and suggest a sensible alternative route rather than opening a case we cannot deliver.
